FACTS, Inc. is an independent Third-Party Administration firm that focuses specifically on qualified retirement plan administration. We work with a variety of investment platforms including brokerage firms, insurance and mutual fund companies, and other providers that offer retirement recordkeeping services. The services provided may vary from company to company.

Most allow for all of the following, but are not limited to:

• Establishing participant accounts
• Website access to plan sponsor for allocation of contributions by type: elective deferral, employer contributions and rollover contributions
• Process name, address and other indicative data changes
• Toll-free Voice Response System (VRS) with customer service representatives available during normal business hours
• Track participants' vested percentage of employer contributions
• Track amounts available for loans or other types of withdrawals
• Processing of loan distributions and repayments
• Processing of participant withdrawals, including applicable federal withholding and mandatory state withholding
• IRS Forms 1099-R and withholding tax remittance and reporting
• Processing of QDRO distributions
• Website accessible to participants to review account information and execute transactions
• Production of quarterly participant statements and reports and transaction confirmations
